Thở máy Ventmed  DS8 không xâm nhập  là biện pháp thông khí không xâm nhập, thường được sử dụng trong các trường hợp hồi sức cấp cứu, đặc biệt phổ biến trong trường hợp hồi sức sơ sinh. Vậy thở máy không xâm nhập là gì? Những trường hợp nào nên và không nên áp dụng phương pháp này? Thở máy không xâm nhập có gây ra những rủi ro nào hay không?

1. Thở máy không xâm nhập là gì?

Máy thở dùng để:

  • Đưa oxy vào phổi và cơ thể;
  • Giúp cơ thể loại bỏ khí CO2 từ phổi;
  • Giảm bớt sức lực cho việc phải hít thở ở những người bệnh gặp khó khăn khi tự thở hoặc gặp vấn đề bệnh lý ngăn cản người bệnh tự thở, khi đó người bệnh sẽ bị khó thở và khó chịu;
  • Giúp người bệnh không thể thở vì chấn thương hệ thần kinh, như não hoặc tủy sống hoặc người có liệt cơ hô hấp

Thở máy không xâm nhập là một phương thức thở bằng máy mà trong suốt chu kỳ hô hấp, bệnh nhân thở tự nhiên nhưng bị đặt một áp lực dương hoặc áp lực dương hai mức.

Phương pháp thở máy không xâm nhập thường được sử dụng cho những bệnh nhân mắc chứng ngưng thở khi ngủ

Áp lực này được tạo ra với mục đích giúp phổi nở, cải thiện quá trình trao đổi khí, từ đó giúp bệnh nhân nhận được lượng oxy cần thiết, đồng thời giảm lượng carbon dioxide và giảm triệu chứng khó thở cho người bệnh.Những bệnh nhân được áp dụng thở máy không xâm nhập bắt buộc phải đang trong tình trạng tỉnh táo và cơ hô hấp vẫn có thể hoạt động bình thường. 

 Thở máy không xâm nhập gồm 2 loại:

+ Máy thở CPAP là loại máy thở chỉ tạo một mức áp lực dương liên tục. 

+ Máy thở BiPAP là loại máy thở có hai mức áp lực dương. 

Các loại máy này đều có kích thước nhỏ gọn, mang đến hiệu quả cao, có thể sử dụng ngay tại nhà với mức chi phí thấp.

Phương pháp thở máy không xâm nhập thường được sử dụng cho những bệnh nhân mắc chứng ngưng thở khi ngủ. Bên cạnh đó, những trường hợp bị suy hô hấp nghiêm trọng có nguy cơ tử vong cao cũng cần được sử dụng phương pháp thở máy không xâm nhập. Cụ thể là những trường hợp dưới đây: 

+ Bệnh nhân bị suy hô hấp do phổi tắc nghẽn mạn tính.

+ Các trường hợp bị phù phổi cấp. 

+ Những người thừa cân béo phì mắc phải hội chứng giảm thông khí. 

+ Bệnh nhân bị viêm phổi

+ Các trường hợp lên cơn hen suyễn cấp tính. 

+ Bệnh nhân thở kém sau phẫu thuật. 

+ Rối loạn nhịp thở do bệnh rối loạn thần kinh. 

+ Các trường hợp bệnh nhân bị suy giảm miễn dịch có triệu chứng suy hô hấp hoặc giảm oxy máu. 

Lưu ý: Một điều kiện quan trọng đó là bệnh nhân cần phải tỉnh táo, đồng thời cơ hô hấp vẫn hoạt động thì mới có thể sử dụng phương pháp thở máy không xâm nhập. Phương pháp này còn mang một ưu điểm là hạn chế những tác dụng phụ và một số nguy cơ biến chứng so với phương pháp đặt nội khí quản hay mở khí quản. 

Sau khi rút nội khí quản, một số bệnh nhân được sử dụng phương pháp thở máy không xâm nhập với mục đích giúp họ hô hấp được bình thường cho tới khi có thể hoàn toàn cai được máy thở.

Một số nguy cơ rủi ro có thể gặp phải khi áp dụng phương pháp thở máy không xâm nhập.

Phương pháp thở máy không xâm nhập có độ an toàn cao, không có nhiều tác dụng phụ và có thể giảm nhiều nguy cơ biến chứng so với một số phương pháp khác như đặt nội khí quản và mở khí quản. Cụ thể là: 

– Bệnh nhân bị kích ứng da, khó chịu do mặt nạ: 

– Đầy bụng ở mức độ nhẹ

– Tình trạng khô miệng, khô mũi: Có thể được khắc phục bằng cách nhỏ nước muối sinh lý, kiểm tra tình trạng rò khí. 

– Sung huyết mũi: Tình trạng này có thể khắc phục bằng một số phương pháp như dùng thuốc chống viêm, thuốc chống dị ứng,…

– Loét cánh mũi có thể do dây mặt nạ quá chặt hoặc dùng loại mặt nạ không phù hợp. Biện pháp khắc phục là nới lỏng dây hoặc thay mặt nạ khác. 

– Da bệnh nhân bị nổi mụn.

– Kích ứng mắt gây đỏ: Với tình trạng này, nguyên nhân cũng có thể là do mặt nạ hoặc dây đai thắt quá chặt. Biện pháp xử lý là nới lỏng dây đai hoặc thay mặt nạ mới. 

– Đau xoang hoặc tai: Những trường hợp này cần giảm áp lực máy cho bệnh nhân

Lưu ý: Dù được thở máy không xâm nhập tại nhà hay tại bệnh viện, bệnh nhân cần tiến hành theo đúng hướng dẫn của bác sĩ.
